What medication management actually is

Medication management is the ongoing clinical relationship that follows a psychiatric evaluation: tracking response, adjusting dose or agent when indicated, watching for side effects, and making sure the treatment plan keeps moving toward what was set out at the evaluation. In a telepsychiatry practice it is delivered by secure video, with the prescribing physician holding each visit personally. It is not a refill line, and it is not a prescription pad, it is a sustained clinical conversation, conducted one patient at a time.

What it looks like at MindGuyMD

Follow-ups are 30 minutes long and are held one at a time, no overlapping slots, no handoffs to a different clinician. Dr. Vinuela reads your chart before each visit and holds each appointment personally, prescribing when clinically indicated. Each follow-up covers how the medication is working, any side effects, and the next concrete step: continue, adjust, taper, transition, or stop. Routine cadence is established after the initial evaluation; urgent questions are routed through Dr. Vinuela's inbox and answered personally as bandwidth allows.

What to expect, visit by visit

    Follow-ups run on a scheduled cadence, not on demand. Routine cadence, typically 4 to 6 weeks to start, is set at the end of your evaluation.

    Each visit is 30 minutes, held privately. Dr. Vinuela reads your chart beforehand, and the time belongs to you alone.

  3. 03

    Each visit produces a specific next step. Continue the current dose, adjust upward or downward, taper, transition medication, or add a second agent, and why.

  4. 04

    Between-visit questions go to Dr. Vinuela's inbox. He answers personally as bandwidth allows; urgent safety concerns go to 911 or 988, never an inbox.

How Dr. Vinuela individualizes the regimen

Medication management isn't a recipe. The dose, the agent, and the duration depend on how you responded to the evaluation, your history, your other medications, and how each change actually feels in your day. Dr. Vinuela is conservative with dose escalation, and explicit about what he and you are watching for between visits, so adjustments are made on response rather than on schedule. If the plan isn't working, the plan changes, not the conversation.

Who this is and is not a fit for

Medication management is appropriate for eligible adult men continuing telepsychiatric care after an individual psychiatric evaluation. It does not include active substance-use detox, chronic pain prescribing, or care for patients located outside New Jersey at the time of the visit.
